How do heartburn medicines work?

The risks associated with these drugs

Kidney disease
A recent study, conducted by a group of experts from the Johns Hopkins Bloomberg School of Health, analyzed two data sets representing more than 250,000 people to find a link between PPI use and kidney problems. Experts found that regular users of PPIs were 13% more likely to develop chronic kidney disease than people with acid reflux who did not take these drugs. Other studies have shown that the use of PPIs increases the risk of chronic renal failure in patients by 20% to 50%.

Cardiac disease
A study, conducted by scientists at Stanford University in California, found that people with gastroesophageal reflux disease (GERD) taking PPIs were at high risk for heart attack. They were also twice as likely to die of cardiovascular disease. Another study found that this type of medicine can lead to cardiovascular problems. The results show that proton pump inhibitors can not only lower gastric acidity but also affect the health of blood vessels. This is mainly because of reduced levels of nitric oxide, a chemical that is produced to protect and relax the blood vessels. This can lead to an increased risk of heart attack.

According to Dr. John Cooke, chair of the cardiovascular sciences department of the Houston Methodist Research Institute, PPI users are 16 to 21 percent more likely to suffer a heart attack over a 17-year period than chronic gastric reflux.

Risk of fracture
In 2010, the US Food and Drug Administration (FDA) issued warnings about the increased risk of osteoporosis and bone fractures associated with the use of PPIs. Use of PPIs of at least 7 years showed that the patient was twice as likely to suffer from an osteoporosis-related fracture. Experts also warn that 5 years of regular use was associated with an increased risk of hip fracture. Other affected areas included the wrist and spine.

Malabsorption of nutrients
Gastric acid is important for the breakdown of food, especially for absorption by the body. Studies have shown that proton pump inhibitors significantly reduce the acidity of the stomach, leading to low absorption of vitamin B12, magnesium, calcium and iron. This can lead to certain health conditions such as:

  • Arrhythmias
  • Muscle spasms
  • Anemia
  • Nervous disorders
  • Psychiatric disorders
  • Dementia
